Ann is very friendly, caring and helpful. As are the nurses and others providing much needed services to the patients. Everyone is very caring, helpful and loving. I’m changing my number to a 10!

Extremely disappointing and deeply concerning experience. I would not recommend Western Slope Health Center to anyone considering it for a loved one. A family member stayed here briefly following a fall and surgery, and the level of care—or lack of it—was extremely troubling. Her condition deteriorated to the point that 911 had to be called from her room, and she was ultimately taken back to the hospital for another medical issue. What made this even more disturbing was witnessing what appeared to be a broader lack of attention to residents. I personally saw residents yelling for help without anyone coming, as well as a woman sitting outside her room for 12+ hours in the same position after vomiting and becoming ill. These were incredibly difficult things to witness. When concerns were brought to Devin, the Facility Administrator, they were denied. A subsequent investigation by the California Department of Public Health (CDPH) found multiple deficiencies at Western Slope Health Center related to the care of residents. This experience completely destroyed my confidence in this facility. When you place someone you love in the care of a healthcare facility, you should be able to trust that they will be treated with dignity, compassion, and appropriate attention. Based on what I experienced and witnessed here, I would strongly urge families to do their research and be extremely cautious before choosing this facility. I would never trust Western Slope Health Center with someone I love again.

The facility was always clean. All of the staff I encountered was very friendly and helpful. I was always greeted when I walked in the door and they even had fresh baked cookies in the lobby. I appreciate all the care my loved one received while there. Thank you to everyone.

Yesterday, Sunday afternoon, I spoke to the receptionist Ashley about possible placement at a skilled nursing facility, and she was phenomenal. She answered all my questions. Addressed all concerns. All while being kind throughout my experience.

What a great visit! I stopped by during one of their community events — they were serving açai bowls for guests, and the energy in the building was contagious. D, the Activity Director, greeted me with so much warmth and enthusiasm — big smile, high energy, made me feel completely at home. Felt like VIP treatment from the moment I walked in! I was there to meet with Jen in Social Services, and as always, the professionalism and genuine care from the team shone through. This is exactly why I feel so confident referring patients here — you can tell the staff truly cares, and that translates into the best possible experience for residents and their families. We loved serving açaí bowls at Western Slope Health Center. The whole process was smooth and the residents and community were so welcoming. A special shoutout to D, the activities director. Clean , great staff happy atmosphere

Did not know what to expect at a rehabilitation facility after my knee surgery. Staff must take happy pills before coming to work as they are so supportive and friendly. Food is served in room or community room. Good food. Physical therapy team is great and prepared me to be able to function at home. My Medicare Advantage type covered all costs. So grateful my choice for rehabilitation was Western Slope.

No care facility is perfect as there will always be disgruntled employees no matter the trade. My experience with the staff here for the most part and been friendly and helpful. My mom seems to be very healthy and they make sure she gets out of the bed and indulges in activities. If I notice something that’s not getting done regularly, I let the staff know and they correct it immediately. It helps if you visit your loved ones often. As for us we see her at least 3 days a week. I’d like to give a shout out to Krystal she has been amazing going above and beyond helping me with my mother’s finances. The staff in general is kind and polite. The PT crews and Jen & Amy are also a lot of help.

My brother was recently transferred to this facility for Hospice care from a hospital. I am extremely impressed with the care he is receiving there, the entire staff has been awesome. I am so thankful he is there!

My mom thrived in here! The staff is top notch! They are kind, patient, understanding, friendly. They have great activities everyday . I’m thankful for this facility and everyone that runs it working to prepare the patients to transition back to their life

I really appreciate all the staff at western slope for there care services and assistance guiding me through my dads end of life care. Its been a long road but they were there to assist us through this difficult process and i would highly recommend this facility to any other families.

Amazing staff!

The folks who actually worked here worked very hard but for the most part this facility was only out to get paid and probably should be audited by the state. We rarely could reach people on the phone, treated patients poorly, would not answer call button. Took 45 minutes to get someone in the room to help my father’s roommate off the floor as he fell. Lost laundry, lack of care overall, I wouldn’t recommend this facility at all
